JOB DESCRIPTION:
Coordinate services in the Children’s Department. Will provide therapeutic services to consumers and their children in the form of groups, individual therapy, activities such as developmental stimulation, recreational/social activities, resiliency and life-skills training and therapeutic tutoring. 

DUTIES AND RESPONSIBILITIES

  1. Develop a comprehensive, individualized treatment plan utilizing available therapeutic resources with measurable goals and objectives identified by the child’s parent(s) and others involved in the child’s life i.e. child welfare, extended family, etc.
  2. Assist in developing and maintaining an innovative children’s program to meet specific needs of children from substance abuse environments.
  3. Provide professional individual, group, educational, play therapy, and parent-child interaction therapy as part of the children’s programming.
  4. Work with Case Manager and other therapists to develop resources and support services for children.
  5. Assist Case Manager in finding appropriate Day Care and Schools for the children.
  6. Acts as advocate between children, parents and community services.
  7. Maintain strict confidentiality concerning all consumers and records protected under Federal Regulation.
  8. Responsible for writing individual and group notes daily on the children and their mothers to monitor progress.
  9. Serves as a member of Treatment Team Staff and presents weekly updates/progress and monthly treatment reviews of each child.
  10. Prepares Discharge Summaries/Aftercare Plans in coordination with child’s mother and her therapist.
  11. Coordinate the development of service for children after they return to the community.
  12. Performs Substance Abuse Counselor duties when so warranted.
  13. Must be familiar with CARF, ODMHSAS, and/or licensing policy, procedures, and regulations.
  14. Perform other assigned duties.

QUALIFICATIONS

  1. Minimum Bachelor’s Degree in social work, counseling, psychology or related field with one-year experience or a Master’s Degree in the above areas with no experience.  Must have a CADAC/LADC certification or working towards CADAC/LADC certification.
  2. Must possess knowledge and ability to effect behavior change through a wide range of treatment modalities.
  3. Must be able to perform group therapy and present educational groups.
  4. Must be computer literate.
  5. Must Possess and maintain a valid State Driver’s License and automobile liability insurance.

Position Title:   Recovery Support Assistant ll

JOB DESCRIPTION:
This position is responsible for maintaining consumer records in the medication log book, observation of self-administered medications, completion of medical intake, documentation of vitals taken, and removing consumers from non-medical detoxification.  This position will also assist consumers with making medical and mental health appointments and transportation as necessary.

DUTIES AND RESPONSIBILITIES:

  1. Complete and monitor consumer records in the Medication Station to assure documentation of forms and records.
  2. Assist consumer with setting medical and mental health appointments.
  3. Responsible for coordinating consumer’s transportation to various appointments, and to work with clinical staff to ensure that appropriate transportation has been arranged
  4. When transporting consumers, will assure all safety rules are followed at all times.
  5. Complete documentation of medical intake, non-medical detoxification, and monitoring of vitals.
  6. Observe and document self-administered medications taken by consumer, maintain accurate count of narcotics on each shift, and report any discrepancies to clinical staff.
  7. Ensure appropriate storage and maintenance of medication, and keep record of all medication belonging to the consumer.
  8. Maintain medical supplies and records in accordance to CARF and/or ODMHSAS regulations.
  9. Maintain strict confidentiality concerning all consumers and records protected under Federal Regulation.
  10. Document initial medical assessments of all new consumers.
  11. Must be familiar with MONARCH, ODMHSAS, and CARF policy, procedures, and/or regulations.

JOB DESCRIPTION:
Responsible for providing supervision of consumers in the residential and community living programs.  Maintain a safe and secure environment during shift assignment.

DUTIES AND RESPONSIBILITIES:

  1. Responsible for supervising and monitoring residential and community living consumer behavior and activities including, but not limited to, consumer chores and offering assistance and instruction during assigned shift.
  2. Provide written reports in the daily log of activities, concerns or problems occurring during shift and chart documentation of any abnormalities and progress of consumers.
  3. Provide a verbal report to the next shift concerning the prior eight-hour activities, concerns and problems.
  4. Responsible for answering the phone and doors, contacting on-call staff when serious problems arise, and other office duties that may develop.
  5. Responsible for preparation and serving of the evening and morning meals using the posted menu, and supervision of clean up after meals by the consumers.
  6. Responsible for maintaining laundry for kitchen and/or assigned facility.

JOB DESCRIPTION:

Will provide therapeutic direct services to consumers in the form of groups, individual therapy, ancillary therapies, education lectures, and psychological assessment, to meet their psychotherapeutic needs while in residential and community treatment.

DUTIES AND RESPONSIBILITIES:

  1. Develop a comprehensive, individualized treatment plan utilizing available therapeutic resources with measurable behavioral goals and objectives identified by the consumer with therapist assistance.
  2. Provides professional individual, group, family therapy and educational groups as part of the consumer’s treatment programming.
  3. Responsible for writing individual and group notes daily on each consumer to monitor progression of treatment.
  4. Serves as a member of Treatment Team Staff and presents weekly updates/progress and Monthly Treatment Reviews and completes Monthly Utilization Reports of each consumer. 
  5. Attends all Team meetings and is a vital component in reviewing potential consumers to determine level of need for residential/community treatment.
  6. Completes Discharge Summary/Aftercare Plan on each consumer to assure discharge criteria and outcomes have been met and devise future recovery plan including making referrals for aftercare services.
  7. Provides and maintains on-going communication with Department of Mental Health, Department of Human Services, Probation and Parole, Court Systems and etc., and write recommendation letters when so warranted.
  8. Maintain strict confidentiality concerning all consumers and records protected under Federal Regulation.
  9. Attend training both with-in facility and community for continued Professional Development.
  10. Meets bi-weekly with Clinical Supervisor for continuous clinical supervision.

JOB DESCRIPTION:
Responsible for the preparation of meals, serving complete meals for large groups, and maintaining appropriate records.

DUTIES AND RESPONSIBILITIES:

  1. Prepare nutritious meals in accordance with menus established by the Licensed Dietitian.
  2. Receive, unpack, inspect, date, and store food supplies and maintain inventory records on all food and kitchen items.
  3. Responsible for accurately checking in inventory as it arrives at the facility. 
  4. Reports any damaged goods and/or shortages to appropriate vendor. 
  5. Decrease waste by determining appropriate quantities of food items to cook.
  6. Responsible for maintaining a good working relationship with various food vendors and/or agencies.
  7. Perform daily inspections of storage areas, take, and record daily temperatures in all cold storage equipment.
  8. Perform all aspects of cleaning, cooking and serving techniques to meet regulatory sanitation, health and safety codes and requirements on a daily basis.  This includes, but is not limited to, the following:  Sweep and mop kitchen floor at the end of the work day; Clean kitchen side of sneeze guard daily; inspect squeeze bottles, keep clean and filled; Keep all kitchen cabinets clean and orderly; Wash morning pots and pans, Clean stove and ovens; Keep counters clean throughout the day; etc.
  9. Supervise consumer cleanup after meals to make sure chores are being completed in a timely manner and being done properly.
